Rarely, primary hyperparathyroidism becomes evident during pregnancy.104 Hyperparathyroidism during pregnancy used to be associated with an increased incidence of fetal death. Perinatal and neonatal complications were also thought to be increased in the hypocalcemic infant whose endogenous PTH production is suppressed by maternal hypercalcemia. Neonatal hypocalcemia and tetany can be the first sign of primary hyperparathyroidism in the mother.
