Mechanism of Action

Ifosfamide is an alkylating agent that is cell cycle-nonspecific. It is an analogue of cyclophosphamide that acts by alkylating DNA and interfering with replication of susceptible cells.


Indications

Third-line therapy for germ cell testicular carcinoma, in combination with other cytotoxic agents. Unlabeled uses: lung, breast, ovarian, pancreatic, and gastric cancer, bone and tissue sarcoma.


Metabolism/Excretion

Extensively metabolized in the liver. Most of the drug is excreted in the urine almost completely unchanged. Half-life: 15 hours.


Dosage Range

♦ Adult: 1.2 g/m2 day for 5 consecutive days. Give repeat cycles every 3 weeks after hematopoietic recovery (platelets >100,000, WBC count >4000). Larger single doses of 5.0 g/m2 can be given over 24 hours. Patients should be vigorously hydrated, PO or IV, with ≥ 2 L fluid per day.
